[Babase] Tooth table docs

There is a general toothnotes column on DARTINGS for anything
leftover that does not fit into the coding scheme.  Susan didn't
seem to think that it was worthwhile having a separate notes
column on TEETH because it's not something that is easily looked
at and because the notes are not particularly amenable to computer
analysis anyway they may as well go in one place.  If necessary
textual remarks can be added to make it clear which notes apply 
to which teeth.
