[Babase] Tooth table docs

Ok, this helps a lot to understand why it looked confusing to us. I'll
keep that in mind and we'll look at all places where these currently
reside.  Please confirm where all those places are and whether
everything relevant has been entered. Combining and being consistent on
whatever we settle on for the condition columns will be essential for
them to be useful.  Then we can have the other range of notes be for
others, perhaps having a 'pointer' from the conditions column---will
think on it, probably can't resolve it  over the weekend but will make
some progress.

This has kind of been my question. Do we combine all this.. leave some
in notes, make large categories for others.   this data is so
inconsistently coded.
